Referrals
Try to help those reading your referrals by writing clearly or, better still, typing the referral.
A software version of the GOS18 is available to use. Simply copy to your hard disc and double click it to start. Use TAB to move through the fields. If you wish to keep each referral on disc you will need to save it with a different name each time. Otherwise, after printing, just close without saving and it will be blank again next time.

Accreditation is by completion of the LOCSU/WOPEC training CD. Full details and an accreditation form are contained in the pack. You should complete and return the form to join the scheme.
Incident Reporting
The PCT collects anonymised data on incidents that may affect patient well-being. The explanatory letter is available here:

Waiting Room Notice
The approved notice for display in the waiting room can be downloaded here in Word format for personalising to your practice. It includes statements about hygiene, complaints and data. It is a requirement of the GOS contract to display an appropriate notice.
GOS Claim Form
This is the new (April 2011) claim form for use whan submitting claims to LASCA.
